Complete the Somerset Health & Wellbeing surve

The Somerset Health & Wellbeing survey is now live and will run until Monday 27th April.  Anyone who is a resident in Somerset and aged 18+ is welcome to take part.

The survey covers a range of topics including physical activity, mental health and wellbeing, smoking and vaping, alcohol and substance use, gambling, oral health, NHS Health Checks and how people access health information.

There is a chance to win a £50 shopping voucher for completing the survey. The survey takes between 15 - 18 minutes to complete.

Chilton Polden is an English civil parish within the unitary authority of Somerset

Chilton Polden is one of a string of villages along the northern slope of the Polden Hills in Somerset. It lies a mile north of the A39 approximately 5 miles east of Bridgwater and 8 miles west of Street. The M5 (4 miles away) makes it a good base for commuters to Taunton or Bristol. The village faces north across the Somerset Levels towards the Mendip Hills and north-west towards the Bristol Channel.
